findOrphans scans linearly on every update, and the orphan lists are never pruned

Where: llviewerobjectlist.cpp:263-275, 2258-2304, 2362-2378; llviewerregion.cpp:2541-2552.
Growth: unbounded when parents never arrive.
Fix: unordered_multimap; purge on region kill.
